Due Diligence Documentation Requirements

You may provide all or part of your documents at this time, or you may provide your documents via email at a later date.

The following documentation is requested in order for Lafayette Federal Credit Union to perform due diligence on your company to be considered for this Indirect Home Improvement Financing Program. Please provide documentation for your business and any closely related affiliate businesses (if applicable).

  1. Audited Business Financial for past two (2) years or signed 4506-T form
  2. Statement of Financial Condition (Year to Date)
  3. Business and Marketing Plan (if available)
  4. All Licensures and Certifications
  5. All Business Insurances
  6. List of Employees with title and job descriptions, and security level access (Security Level Access includes examples such as: keys to office/may enter building at any time, level of access to all pertinent software/data of sensitive customer information – restricted or all access, need for username and password to LFCU Origination Software y/n?)
  7. Key employee(s) resume(s)
  8. List of major suppliers that you do business with
  9. List of business references from current comparable business relationships (preferably within the financial lending sector)
  10. Business Organizational Chart
  11. Chart of Business Ownership(s) (Names and % of ownership)
  12. Credit Profile of Customer (typical loan amount, term requested, FICO. Etc.)

The named principals, owners, and partners instruct any third party including but not limited to any consumer reporting agency, partner, and vendor to provide such report.
